Mitotane可視為一種腎上腺細胞毒性藥物(cytotoxic agent)，可在不破壞細胞的情況下抑制腎上腺；其作用的生化機轉仍未清楚。現有的資料顯示該藥物不但調節周邊組織的類固醇代謝，而且也能直接抑制腎上腺皮質。Mitotane可改變人體內腎上腺皮質內泌素(corisol)在腎臟以外的代謝；雖然皮質類固醇的血漿中濃度沒有下降，但17-羥皮質類固醇(17-hydroxy corticosteroid)的測定值則降低。該藥物顯然會增加6-B-羥基腎上腺皮質內泌素(6-B-hydroxyl cortisol)的形成。Mitotane is an inhibitor of the adrenal cortex.
