ch30Neuropathic pains arise from an injured pain transmission ormodulation system. Surgical intervention or trauma is frequentlythe cause. For example, trauma to the infraorbital region may leadto numbness or pain in the distribution of the infraorbital nerve. Indental surgery, extraction of impacted mandibular third molars carriesa measurable risk of nerve damage to the mandibular and lingualnerves. In the majority of these cases, damage leads to paresthesia,an abnormal sensation in the dermatome of the affected nerve.Typically this sensation is one of mild numbness or tingling. Lossof all sensation may occur when the nerve is transected. In a subsetof cases, dysesthesia—an abnormal, unpleasant sensation—canresult; it is often described as a burning or sharp electric shocklikesensation. When a patient complains of burning or sharp shocklikepain in the face or mouth, pain of neuropathic origin shouldbe included in the differential diagnosis. One should appreciatethat the oral cavity is the most common site of amputation, ifone recognizes amputations to include the teeth and the dentalpulp (i.e., endodontics).
