【評論內容】這題A B C 都出於 Newman and Carranza's Clinical Periodontology 13edition p.616 的一整段課文原本是在講 Healing After Gingivectomy不知道為什麼出題老師要改成 healing after periodontal surgery這段內容跟 常考的flap surgery的 healing stage有一點不同(然後仔細看課文會發現 (C) 也很有問題 = =)先看 (A)課文說 ginigivectomy 術後 傷口的第一個反應就是在表面形成血塊而血塊底下 會包覆 急性發炎病壞死的組織血塊之後會被 granulation tissue取代而發炎壞死的組織底下 會開始出現 成血管細胞 (angioblasts)接著fibroblast 也出現 大家開始努力 進行血管再生成 組織修復等等12-24小時後 上皮會開始遷移過來 將肉芽組織與外界污染隔離→ (B)對因此推論可知 → 血塊如果太厚 整個傷口之後 就都會是 肉芽組織 無法正常進行血管再生成及組織修復 之後也會被虎視眈眈的上皮遷移而取代而無法形成良好具血管化的結締組織 表面血塊在此有點類似membrane的作用 (下圖解)The initial response after gingivectomy is the formation of a protective surface blood clot. (表面血塊)The underlying tissue becomes acutely inlamed with necrosis. The clot is then replaced by granulation tissue. (一系列修復反應)(B) 正確 以下原文After 12 to 24 hours, epithelial cells at the margins of the wound begin to migrate over the granulation tissue, thereby separating it from the contaminated surface layer of the clot.(C) 十分詭異 = = 以下原文直翻 絕無刪減、節錄5~14天之後 表面上皮癒合完成 (注意 只有表面)前4週的 上皮角化程度 還無法恢復成術前的狀態完整的上皮形成癒合 → 一個月 ( 2week 上皮表面癒合 1month 上皮完整癒合)上皮的血管系統 → 16天 才恢復正常 結締組織完全癒合 → 至少 7 週 根據上段 最快也要 16 天 血管化才正常而 上皮底下的結締組織 也要 7 週的時間長選項(C)的12天最快也只有上皮表面癒合 完整的上皮也要一個月才長好After 5 to 14 days, surface epithelialization is generally completeDuring the first 4 weeks after gingivectomy, keratinization is less than it was before surgery. Complete repair of the connective tissue takes about 7 weeks(D) 實在太常考了 8-12週的海綿骨可作為BONE GRAFT來源o.s出題的選項 常集中在課本某一段落個人堅信這題 ABC 都是這段課文來的但不知為何(C)的內容如此奇怪其他題 最常考的都是：上皮 2週表面癒合+再血管化 / 4週完全癒合所以這題的12天就不要理他了註：如果用 " 10 to 12 days " 當關鍵字搜尋在Newman and Carranza中 只會找到 牙齦的有絲分裂週期 大約為10-12天如果本意是想考這個我也是跪了哈哈
