【評論內容】幫忙整理 koda & kimble p. 559 休克治療內文septic shock的治療中可考慮單用norepinephrine 或並用 強心劑在一篇收錄血液動力學不穩定的shock病人，比較dopamine or norepinephrine的療效療效指標：保持6hr以上血液動力學與氧氣的穩定達標率：dopamine (31%) vs norepinephrine (93%)在相同的study中，使用dopamine/dobutamine未能達標者，使用norepinephrine皆顯著扭轉低血壓、也顯著增加MAP、SVR、尿量 studies indicate that norepinephrine alone or in combination with inotropic agents can be beneficial in the management of septic shock Martin et al. compared the ability of dopamine and norepinephrine to reverse hemodynamic and metabolic abnormalities of hyperdynamic shock.76 They prospectively randomly assigned 32 volume-resuscitated patients with hyperdynamic sepsis to receive either dopamine or norepinephrine with the goal of achieving and maintaining normal hemodynamic and oxygen transport parameters for at least 6 hours. If goals were not achieved with one agent, the other agent was added. With the use of dopamine, 31% of patients met the predefined therapeuticgoals. In contrast, 93% of patients treated with norepinephrine met the predefined goals.In a similar study of patients in whom previous therapy with plasma volume expansion and dopamine or dobutamine had failed, norepinephrine reversed hypotension and significantly increased MAP, SVR, and urine output
